Kör alakú, két oldalas sport emlékérem.
A: középen jobbra néző bajuszos férfifej. Domború köriratban, csillagokkal elválasztva "VITÉZ LEVELDI KOZMA MIKLÓS".
B: jobb oldalon a lakihegyi szivarantenna és egy tollszár szalaggal és filmszalaggal átfogva; bal oldalon domború betűkkel, három sorban "M.T.I. és társ- / vállalatainak / sportegyesülete". Alul, szintén domború betűkkel: "Boldogfai Farkas"
