Az okmány a nagykárolyi ref. egyházmegye 1907. okt. 3-i mátészalkai közgyűlési jegyzőkönyvének kivonata. 11. szám alatt felsorolja az új esküt tevő tanítókat. (Szoboszlay Piroska, Borbáth Andor, Melegh Dániel, Sepsey István). A kivonatot Borbáth Andor (András más iratokban) mátészalkai tanító részére állították ki. Alul a nagykárolyi ref. egyházmegye esperesi hivatalának bélyegzője, ill. Berey József esperes aláírása látható.
