Die Plakette ist aus hellem Holz auf dem ein Metallschild befestigt ist. Der Untergrund des Schildes ist rot, mit weißer Schrift: "Schöner unsere Städte und Gemeinden - Mach mit" als Umlauftext. In der Mitte des Schildes steht ein stilisierter Baum mit Hammer und Sichel; darunter Papieraufkleber mit Schreibmaschinentext: Dem 2. Sieger im Wettbewerb zu Ehren des 24. Jahrestages der DDR
