ἡ ἁγία τοῦ θεοῦ ἐκκλησία Ἑρμουπόλεως διὰ τοῦ ἐνδοξοτάτου Ἰωαννακίου ἰλλουστρίου καὶ φροντιστοῦ τῆς αὐτῆς ἐκκλησίας Ἑρμουπόλεως διὰ Κολλούθου ἀπαιτητοῦ μερίδος Ἑρμουπόλεως τοῖς κληρονόμοις Ζαχαρία δημότουδημοτη διὰ τοῦ κυρίου Κολλούθου προνοητοῦ. ἐδεξάμην καὶ ἐπληρωσάμην παρὰ σοῦ ὑπὲρ ἐμφυτεύματος καραπῶν πρώτης ἰνδικτίονος ὑπὲρ τοῦ σοῦ μέρους χρυσοῦ νομισμάτιον ἓν εὔσταθμον κεράτια ἑπτά, γίνεται χρυσοῦ νόμισμα α κεράτια ζ, καὶ πρὸς σὴν ἀσφάλειαν πεποίημαι τὴν πληρωτικὴν ἀπόδειξιν ὡς πρόκειται. ἡ ἁγία τοῦ θεοῦ ἐκκλησία Ἑρμουπόλεως διʼ ἐμοῦ Κολλούθου ἀπαιτητοῦ, στοιχεῖ μοι ἡ ἀπόδειξις ὡς πρόκειται.
