Die Plakette ist wie ein Wappen geformt und gelb. Ein schwarzer Rand ist zu erkennen. Mittig in zentriertem Format mit schwarzer Schrift steht: " Kreis- // Heimat- // Fest // Wolmirstedt // v. 23.8.-26.8. // 1957 "
Auf der in grau gehaltenen Rückseite ist in schwarz aufgedruckt: Nr. 28678.
Die Plakette hat im oberen Teil ein Loch.
