Description

Blatt im Hochformat. Im oberen Teil die Adresse der praxis eingedruckt und mit Linie darunter abgeschlossen. Unter der Linie steht "RP, gedruckt" und das Datum handschriftlich. Darunter ist die Arzei Belladonna Tabletten und darunter die Darreichungsmenge (Dosis) vermerkt. Das Rezept trägt die Unterschrift von Sr. Spitznagel.
Das Rezept ist nummeriert und trägt den Nummernstempel "177". Darunter ist der Stempel der "Pohle´schen Apotheke in Sinzig aufgedruckt.
