Diese Medaille wurde zum 25. Jahrestag der Befreiung von Cherson hergestellt.
Auf dem Revers steht auf ukrainisch: "Zum 25. Jahrestag der Befreiung von Cherson von deutschen faschistischen Angreifern am 13. März 1969".
Auf dem Avers wird das Gesichtsprofil eines sowjetischen Soldaten dargestellt. Im Hintergrund ist das Gebäude vom Kunstmuseum in Cherson erkennbar.
